There are more factors that contributes to an individual’s health outcome than just medical care. This first of two posters seeks to remind healthcare providers of the definition of social determinants and highlight some common challenges faces by particularly vulnerable populations.

This second of two posters on SDOH seeks to introduce practitioners to the small, actionable steps that can be incorporated by a healthcare team on a patient, practice, and community level.

More than My Diagnosis

For some people, being diagnosed with a mental health disorder can be empowering, and for others, stigmatizing.  The individuals we spent time with for this initiative made it clear to us that while living with a mental health disorder may be part of their identity, their lives are not lived through the lens of a diagnosis.
